Person Directed Planning Initiative
This program prepares for life transitions and help clients create a meaningful life in their community through finding volunteer positions, employment and/or educational opportunities.
Services
- Work to understand the client’s abilities and interests to connect them to their community
- Identify resources in the community that could contribute to the client’s success
- Help build a network of relationships to support the client in achieving their goals
- Increase the opportunity for individuals with developmental disabilities to participate in decision-making about the supports they receive and the help they need.
- Create a step-by-step plan for participation in the areas of employment, education and community engagement.
